Do I need a referral?

Yes. For parents seeking occupational, physical, speech, and developmental therapy services for their child, a referral from the child’s primary care physician or attending physicians is required before an assessment can be completed. FDDC provides the referral paperwork that must be completed by the physician. Please pick up a referral packet from FDDC prior to your physician appointment.

Is there a cost for services?

Early Intervention and Adult Development Day Treatment services, including transportation, are funded through Medicaid and are provided at no cost to parents of children and adults who qualify during program hours of 8:00am-3:00pm. Please contact FDDC for information regarding traditional daycare rates and services outside of program hours.

How long does it take to get started?

The timeline for beginning servies may vary depending on eligibility requirements, completion of referral paperwork, and the assessment process. FDDC staff will guide families through each step and work to begin services as quickly as possible.

What ages do you serve?

FDDC serves children ages 6 weeks to 5 years old and adults 18 and above. Services and programs are based on eligibility requirements and individual needs.

What types of services do you offer?

FDDC offers a variety of services designed to support children and adults with developmental disabilities. Services may include Early Intervention Day Treatment and Adult Development Day Treatment programs, occupational therapy, physical therapy, speech therapy, transportation services, and traditional daycare options, and nutritious meals and snacks. Services are based on eligibility requirements and individual needs.

What happens after I submit the form?

Once your form has been submitted, an FDDC team member will review your information and contact you regarding the next steps. This may include discussing eligibility requirements, completing needed paperwork, obtaining physician documentation, and scheduling assessments if applicable.

We understand that choosing the right program can feel overwhelming. Our FDDC team is here to help guide you through the process, answer your questions, and help determine which services and programs best meet your child’s or family member’s needs.
